How do Graduates Obtain Licensure in the State of Georgia?

To obtain a license in the state of Georgia, a graduate must first download the appropriate application from the Georgia Medical Board website (www.medicalboard.state.ga.us) or directly contact the board at (404) 656-3913. The application contains several sections including the following:

  • Pages 1-3–Physician Assistant Application
  • Page 4–Utilization of physician assistant (to be filled out by your supervising physician)
  • Form A –Affidavit of Applicant (needs to be notarized)
  • Form C or Form D–2 current references (licensed Physicians- other than those who are going to supervise you)
  • Form F– Job description -This is a primary care job description. It also includes prescribing privileges and the right to sign for and distribute professional samples. If you would like to request any other procedures that may not have been taught in the PA curriculum and/or will not be done in the presence of your physician, the applicant must complete an Extra Duties Request form to be approved.
  • Form I–Verification of Licensure Form- for those PAs who have practiced in other states
  • Form K–Certificate of Education for Physician Assistants-The graduate´s school will fill out this section and mail it directly back to the Georgia Medical Board
  • Resume or Curriculum Vitae– Send in your current resume or CV

Once the application is completed and returned to the board with a $200.00 initial application fee, the PA‘s file will be reviewed within 5-10 business days. The applicant will be notified in writing of the initial determination and the next scheduled Medical Board meeting date which occurs monthly. At this time while the PA may be eligible for a temporary permit. These permits are granted to those who have otherwise met the requirements for Board licensure and who have either applied to take the next available examination or has already taken the examination and is awaiting the results. There is no charge for a temporary permit under those circumstances. The temporary permit expires once the applicant has received their permanent license or upon notification of the applicant´s failure of the PANCE exam.
